چکیده

Endoscopic sphincterotomy (EST) is a commonly used technique for retrieval of common bile duct (CBD) stones. The short-term complications of this technique can include bleeding, perforation, pancreatitis, and cholangitis, and were reported to have a 10% occurrence rate in a multicenter study involving 2347 patients. Endoscopic papillary balloon dilation (EPBD) using balloon diameters of 6e10 mm was first reported by Staritz et al as an alternative endoscopic technique for EST. However, the extent to which EPBD is effective and safe for removal of CBD stones remains controversial. In this issue of the Journal of the Chinese Medical Association, Tsai et al evaluated the clinical efficacy of EPBD in patients with recurrent CBD stones after previous EST. Accordingly, we have reviewed some of the commonly-discussed issues about EPBD versus EST as they relate to the article.

Comparison of the usefulness of endoscopic papillary large-balloon dilation with endoscopic sphincterotomy for large and multiple common bile duct stones

BACKGROUND Endoscopic sphincterotomy (EST) is currently recognized as the primary endoscopic treatment for common bile duct stones. However, it is difficult to remove multiple (≥ 3) or large (≥ 15 mm) common bile duct stones with EST alone. Recently, EST plus endoscopic papillary large-balloon dilation (EPLBD) was reported to be an effective treatment for such bile duct stones. We compared the ...

Current status of endoscopic papillary balloon dilation for the treatment of bile duct stones

BACKGROUND/PURPOSE While endoscopic sphincterotomy (EST) is performed worldwide for the removal of common bile duct stones, many biliary endoscopists hesitate to regard endoscopic papillary balloon dilation (EPBD) as a standard procedure for treatment. Therefore, the aim of this review is to re-evaluate the status of EPBD for the treatment of common bile duct stones. RESULTS A major benefit o...

Complete endoscopic sphincterotomy with vs. without large-balloon dilation for the removal of large bile duct stones: randomized multicenter study.

Background and study aims Endoscopic sphincterotomy plus large-balloon dilation (ES-LBD) has been reported as an alternative to endoscopic sphincterotomy for the removal of bile duct stones. This multicenter study compared complete endoscopic sphincterotomy with vs. without large-balloon dilation for the removal of large bile duct stones. This is the first randomized multicenter study to evalua...

Endoscopic Papillary Balloon Dilation with Large Balloon after Limited Sphincterotomy for Retrieval of Choledocholithiasis

Endoscopic papillary balloon dilation (EBD) for choledocholithiasis is known to be comparable to endoscopic sphincterotomy (EST) especially in cases of small stones. With larger stones, EBD with conventional balloon, which have a diameter of 6-8 mm, was reported as less effective for extraction of stones. We evaluated the efficacy and complications of EBD with large balloons (10-15 mm) after li...

Which Is the Better Treatment for the Removal of Large Biliary Stones? Endoscopic Papillary Large Balloon Dilation versus Endoscopic Sphincterotomy

BACKGROUND/AIMS We evaluated the efficacy and cost-effectiveness of endoscopic papillary large balloon dilation (EPLBD) for large common bile duct (CBD) stone removal compared with endoscopic sphincterotomy (EST). METHODS A total of 1,580 patients who underwent endoscopic CBD stone extraction between January 2001 and July 2010 were reviewed. The following inclusion criteria were applied: chol...
